Relação entre disfunção temporomandibular e mordida cruzada posterior / Relationship between temporomandibular disorders and posterior crossbite

Resumo

Objetivo: Avaliar clinicamente os sinais e sintomas da disfunção temporomandibular em crianças com mordida cruzada posterior. Métodos: Foram avaliadas 456 crianças com idade entre sete e doze anos de idade na dentição mista, sendo selecionados 42 pacientes que apresentavam a maloclusão mordida cruzada posterior. O exame clínico constituiu-se da observação de hábitos parafuncionais e presença ou ausência de sinais clínicos da disfunção temporomandibular nos pacientes portadores de mordida cruzada posterior. Os pacientes foram submetidos a uma entrevista, supervisionada pelos pais, cujas perguntas relacionavam-se com a frequência dos sinais e sintomas da disfunção temporomandibular. Resultados: O hábito de sucção digital e/ou chupeta (80,95%) e onicofagia (57,14%) foram os hábitos parafuncionais mais prevalentes. O sinal clínico de disfunção temporomandibular mais encontrado foi o ruído articular (21,42%) e as sintomatologias da disfunção temporomandibular mais registradas foram dor de cabeça (59,25%) e cansaço ou dor muscular ao mastigar (38,09%). Conclusão: Os pacientes com mordida cruzada posterior apresentaram alta incidência de sintomatologia de disfunção temporomandibular, sendo os sinais e os sintomas mais encontrados, a dor de cabeça e cansaço muscular. Entretanto, não se pode afirmar até que ponto à mordida cruzada posterior é considerada um fator predisponente ou apenas coincidente.

Abstract

Objective: The objective of this study was to evaluate the clinical signs and symptoms of temporomandibular disorders in children with posterior crossbite. Methods: A total of 456 children aged between seven and twelve years of age with mixed dentition were assessed. Of these, 42 patients with posterior crossbite malocclusion were selected. Clinical examination consisted of observation of parafunctional habits and presence or absence of clinical signs of TMD in patients with posterior crossbite. The patients were interviewed in the presence of their parents to determine the presence of signs and symptoms of TMD and their frequency. Results: The results showed that finger sucking and / or use of pacifiers (80.95%) and onychophagy (57.14%) were the most prevalent parafunctional habits. The most common clinical sign of TMD was joint noise (21.42%) and the most common symptoms were headaches (59.25%) and fatigue or muscle pain while chewing (38.09%). Conclusion: Patients with posterior crossbite present a high incidence of TMD symptoms. The most common signs and symptoms were headache and muscle fatigue. However, it is not possible to state the extent to which posterior crossbite is a predisposing factor or merely coincidental.
